Please print out, complete, and sign this form, one for each camper aged 10–17. Submit all completed forms to the FAC office with final payment before arrival at camp.

AGREEMENT

I want to be a camper at the Folk Arts Center of New England’s international folk dance and music session at Pinewoods Camp. I have read, and agree to abide by, “Rules for Campers Under 18” and “Policies for Families and Children.” I agree to participate to my fullest in the programs offered. I will do my best to make this a good experience for myself and my fellow campers. I understand that failure to follow camp rules or safety regulations may result in my dismissal from camp without a refund.
